D-dimer (or D dimer) is a fibrin degradation product (or FDP), a small protein fragment present in the blood after a blood clot is broken down by the body’s own clot dissolving mechanisms called fibrinolysis. It is so named because it contains of two cross-linked D fragments of the fibrin protein. Under normal circumstances, D-dimer is not present in the blood or if it is, it is at very low levels. D-dimer concentration may be determined by a blood test and it is raised in thrombosis or clot formation. Since its introduction in the 1990s, it has become an important test performed in patients with suspected thrombotic disorders such as Deep Vein Thrombosis and Pulmonary Embolism. While a negative result practically rules out thrombosis, a positive result can indicate thrombosis but does not rule out other potential causes. So the D-dimer blood test used to exclude thromboembolic disease where the probability is low. Doctors may request a D-dimer when a patient has symptoms of DVT, such as leg pain, tenderness, swelling and discolouration or when a patient has symptoms of Pulmonary Embolism, such as breathlessness, cough, and sharp chest pains. D-dimer is especially useful when the doctor thinks that something other than DVT or PE is causing the symptoms. It is a quick, non-invasive way you’re your doctor to help exclude abnormal or excess clotting. When a DVT is suspected, the D-dimer blood test is used as part of the Wells Scoring system. In people thought to be at low risk, a normal D-dimer blood test excludes the diagnosis. The coagulation cascade has evolved as an essential defense mechanism for maintaining hemostasis during tissue or blood vessel injury (Nikolajsen 2014) Activation of the cascade ultimately results in the formation of an insoluble blood clot (Nikolajsen 2014). The clot consists mainly of fibrin fibers, platelets and red blood cells (Nikolajsen 2014). The fibers are formed when activated thrombin cleaves fibrinogen into fibrin monomers, which then assemble into fibrin fibers (Nikolajsen 2014). To obtain fibers that are capable of resisting mechanical shear and proteolytic degradation, concurrent cleavage of coagulation factor XIII (FXIII) by thrombin is needed (Nikolajsen 2014). Once cleaved and activated, FXIIIa is able to stabilize the fibrin fibers and alter the characteristics of the clot (Nikolajsen 2014). Although activation of the coagulation cascade and consequently FXIIIa is essential during blood vessel damage, it may contribute negatively in pathological situations such as sepsis and cardiovascular disease (Nikolajsen 2014). FXIIIa belongs to the family of human transglutaminases (TG) (E.C 2.3.2.13) (Nikolajsen 2014). This group of enzymes is defined by their common ability to catalyze thiol- and Ca2+- dependent transamidation reactions (Nikolajsen 2014). The TG-catalyzed reaction between specific Gln and Lys residues within substrate proteins results in the formation of covalent cross-links, defined as ε-(γ-glutamyl)lysine isopeptide bonds (Nikolajsen 2014). The major physiological function of FXIIIa is to stabilize the fibrin clot during coagulation (Nikolajsen 2014). FXIIIa cross-links α- and γ-chains of fibrin monomers within fibrin fibers once these have been formed (Nikolajsen 2014). The inter-molecular cross-linking of the fibrin monomers converts loose fibrin fibers into a stable fiber network that is able to withstand mechanical stress and resist clot rupture (Nikolajsen 2014). FXIIIa deficiency, which causes severe bleeding diathesis, demonstrates the essential role of FXIIIa in coagulation (Nikolajsen 2014). This can lead to thrombosis in subacute or chronic form, but more often bleeding occurs when the coagulation factors become depleted. Disseminated intravascular coagulation is characterized by evidence of thrombin and plasma activation. Disseminated intravascular coagulation is classically characterized by the concurrent appearance of generalized vascular coagulation deposition, which jeopardizes adequate blood supply to different organs and thereby contributes to organ failure. Due to the continuous activation of the coagulation system and other factors, such as altered synthesis and increased degradation of coagulation proteins and protease inhibitors, platelet excretion may occur, which may cause major bleeding in several places. In addition, high levels of fibrin degradation products may affect the function of platelets and the crosslinking of fibrin and therefore contribute to the tendency to bleed. Epidemiology The conditions arise in response to another pathology rather than as a major event. There are no predisposition factors in age, sex or race. Risk Factors Conditions that may be complicated by the spread of intravascular coagulation include: • Infections, especially septicemia, typhoid fever, rocky mountain fever and parasites. The release of meningococcal pepticemia is classical. • Malignancy, especially leukemia. • Greater trauma that includes crush syndrome and sometimes burns. • Some connective tissue diseases that include antifosfolipid syndrome. • Transfusion of incompatible blood. • Heat stroke. • Dissection of the aortic aneurysm. • Some snake bites Presentation The immediate apparent properties are usually the underlying conditions that have caused disseminated intravascular coagulation, especially if it is acute. In addition, there may be large bruises or spontaneous bleeding at the venipuncture sites, the soft palate, the legs and the trauma. In subacute or chronic disseminated intravascular coagulation, the functions may be thrombotic rather than signs of venous thrombosis. In the acute situation there are many properties that can be found: • Bleeding from at least three unrelated sites is typical and likely places include: • Ear, nose and throat • Gastrointestinal tract • Airway • Venepuncture or infusion fluid • Confusion or disorientation. • Fever. • Signs of bleeding. • Signs of anxiety disorder in adults (ARDS). • The skin can show several characters that include: • Petechia • Purple • Hemorrhagic noise • Acralcyanosis • Scalp necrosis of the lower leg (fulminating purpura) • Signs of thrombosis • Local infarction and gangways Diagnosis Blood tests can show that the number of platelets in a blood sample has decreased. Platelets are used when blood clots and blood take a long time to coagulate. The diagnosis of disseminated intravascular coagulation is confirmed if the results of the test show abnormally increased amounts of D-Dimer in plasma, a substance that releases thrombocytes when decomposed; More D-Dimer indicates that more blood clots occur than usual and often a low or decreasing level of fibrinogen. A protein consumed by blood clots. Treatment The underlying disease must be identified and corrected, whether it is an obstetric problem, an infection or a cancer. Treatment of coagulation problems when the cause is corrected. Disseminated intravascular coagulation that develops suddenly is life threatening and treated as an emergency. Transglutaminases catalyzes the formation of ε -(γ-glutamyl)lysine bonds between a selected glutamine residue of one protein and a lysine residue of another protein, leading to protein crosslinking (Remon Van Geel 1982). BETWEEN GLUTAMINE AND LYSINE Transglutaminases catalyzes post-translational cross-link reactions between two substrates that can be two proteins or two residues of the same protein, involving lysine and glutamine (Serafini-Fracassini and Del Duca 2008). The transglutaminases are a family of structurally and functionally related enzymes which catalyze Ca2+-dependent post-translational modification of proteins by introducing a protein-protein crosslink between a specific glutamine and a lysine residue (Klöck et al. 2012) Transglutaminase is an enzyme that catalyzes the formation of isopeptide cross-links between glutamine and lysine residues in a variety of proteins (Sugimura 2006). RESULTING BOND; A COVALENT BOND TERMED "ISOPEPTIDE BOND": The cross-link formed is called an ε-(γ-glutamyl)lysine isopeptide bond (Lantto 2007). The TG catalyzes the formation of ε-( γ-glutamyl) lysine bonds (isopeptide bond) between peptide-bound glutamine residues and the primary amine group of various amines (Haroon 1999). The ε-(γ-glutamyl)lysine isopeptide bond is highly resistant to proteolysis and denaturants and it gives stable, rigid and insoluble protein complexes (Sulic 2011). The resulting bonds are covalent, stable, and resistant to proteolysis, thereby increasing the resistance of tissues to chemical, enzymatic, and physical degradation (Greenberg 1991). These isopeptide bonds are stable and more resistant to proteolytic degradation than noncovalent linkages. The covalent cross-linking reaction increases the resistance of proteins to chemical, enzymatic, and physical disruption (Haroon 1999). The thrombin time (TT), also known as the thrombin clotting time (TCT) is a blood test that measures the time it takes for a clot to form in the plasma of a blood sample containing anticoagulant, after an excess of thrombin has been added. It is used to diagnose blood coagulation disorders and to assess the effectiveness of fibrinolytic therapy. This test is repeated with pooled plasma from normal patients. The difference in time between the test and the 'normal' indicates an abnormality in the conversion of fibrinogen (a soluble protein) to fibrin, an insoluble protein. The thrombin time compares the rate of clot formation to that of a sample of normal pooled plasma. Thrombin is added to the samples of plasma. If the time it takes for the plasma to clot is prolonged, a quantitative (fibrinogen deficiency) or qualitative (dysfunctional fibrinogen) defect is present. In blood samples containing heparin, a substance derived from snake venom called batroxobin (formerly reptilase) is used instead of thrombin. Batroxobin has a similar action to thrombin but unlike thrombin it is not inhibited by heparin. Normal values for thrombin time are 12 to 14 seconds. If batroxobin is used, the time should be between 15 and 20 seconds. Thrombin time can be prolonged by heparin, fibrin degradation products, and fibrinogen deficiency or abnormality. After separating the plasma from the whole blood by centrifugation, bovine thrombin is added to the sample of plasma. Clot formation is detected optically or mechanically by a coagulation instrument. The time between the addition of the thrombin and the clot formation is recorded as the thrombin clotting time. Whole blood is taken with either citrate or oxalate additive (if using the vacutainer system, this is a light blue top tube). As with other coagulation assays, the tube must not be over- or under-filled in order to ensure the correct anticoagulant-to-blood ratio: one part anticoagulant per nine parts blood. The reference ranges of the thrombin clotting time is generally 22 seconds, and from 14 to 16 seconds. Laboratories usually calculate their own ranges, based on the method used and the results obtained from healthy individuals from the local population. Separate ranges are used for infants. Blood samples that are more than eight hours old can give inaccurate results when tested.

Collagen has important applications in medical applications for the healing of wounds. Chronic, non-healing wounds include diabetic foot ulcers and pressure ulcers (Frykburg & Banks, 2015). An estimated 6.5 million people suffer from chronic wounds in the United States, and they must rely on medical procedures for effective healing (Sen, Gordillo, Roy, Kirsner, Lambert, Hunt, Gottrup, Gurtner, & Longaker, 2004). Many patients, however, have to deal with the financial burden, as many current treatment options are expensive, ineffective, and inconvenient. To improve the current standards, engineers must face the technological challenge of selecting strong, nontoxic, biocompatible, and effective wound healing biomaterials; this is where collagen comes in. Collagen, a naturally-occurring matrix polymer, embodies the desired characteristics for an improved medical device, and also provides an effective treatment for non-healing wounds. Since chronic wounds have excess matrix metalloproteinases (MMPs), which degrade both viable and nonviable collagen used in the wound healing process (Ruszczack, 2003), providing a collagen-based wound scaffold compensates for the loss of collagen required for proper tissue regeneration (Tamariz & Grinnel, 2002). Engineers can vary collagen’s density, packing, and orientation to produce various mechanical properties for a wide range of medical applications. Depending on its processing, collagen may have some limitations, including altered cell growth or movement, the consequential development of adverse mechanical properties, and/or polymer shrinkage (Cuy, 2004). However, combining collagen with synthetic and/or biological materials alters collagen interactions with the skin and eliminates undesired limitations (Cuy, 2004), ultimately making the material a -- theoretically -- useful means for effective wound care. The applications of collagens in wound healing are immense. As collagen has been found to have a bulk density of 0.01 to 0.3 g/cm3 (Ksander & Ogawa, 1988), an implant can be small and unobstructive to the patient. Due to the low bulk density, patient comfort can be maximized, while effectively facilitating the growth of connective tissue and angiogenesis. The structure of collagen also lends to its efficacy, namely the development of many of its properties, such as adhesiveness, density, and toughness. Collagen can be processed and derived from various animals.
